Medicare Referrals

Thank you so much for referring your patient to The Physiology Collective for management of their chronic disease. TPC has experienced Exercise Physiologists with a keen interest in treating the following: 

  • Cardiovascular disease (including rehabilitation post cardiac event) 
  • Orthopedic conditions and injuries (including pre and post op knee replacements, hip replacements) 
  • Obesity related diseases 
  • Neurological diseases (including MS, Parkinson’s Disease) 
  • Diabetes Management 
  • Oncology (during and post treatment) 

We have a clinic located at 1A/ 200 Preston Road, Manly West, but we are also able to provide home visits (available on request and travel charges apply), as well as Telehealth appointments. TPC has on the spot claiming in clinic as well as Private Health Insurance rebates following completion of the Medicare referral. 

We also offer the transition into small group exercise classes (if appropriate) following completion of the CDMP to assist with affordability and motivation. 

Price List:

  • Initial Assessment: $140 ($61.80 medicare rebate) 
  • Follow up (40 minutes): $95 ($61.80 medicare rebate)
  • Group Classes: $22 (Medicare rebate unavailable- Prviate health insurance can be used if you have cover)
